Abstract | 近年,フレイル,サルコペニアが癌の治療成績に影響するという報告が多く見られる.身体的フレイルの重要な要因がサルコペニアであり,慢性炎症が原因となる.慢性炎症にはDAMPs(Damage‐Associated Molecular Patterns)が関与しており,circulating cell‐free DNA(ccfDNA)もDAMPsの一種であると考えられている.われわれは,ccfDNAの断片化の程度を測定し,周術期サルコペニア,ccfDNAと癌の治療成績の関連を検討した.術後半年後に撮影した腹部CT検査でPMI(Psoas Muscle Index)を測定したところ術後合併症を発症した群ではPMIが低値で,無再発生存期間が不良であった.また,術前にccfDNA LF(Long Fragment)を認めた症例では無再発生存期間が不良であった.術後合併症を発症しないことが重要であり,そのためにはサルコペニア,慢性炎症への対策が非常に重要であると考えられる. |
