副咽頭間隙に原発し遠隔転移した多形腺腫例
症例は25歳男性, 左副咽頭間隙に発生した巨大な耳下腺深葉由来の多形腺腫を頸部外切開にて摘出した.約9ヵ月後, 頸椎C5/6に新たに腫瘍が発見された.頸椎腫瘍摘出術をうけたところ, その病理検査結果も多形腺腫であり, 悪性像は見られなかった.症例は転移病巣摘出術後約4ヶ月, 広範な両側の肺腫瘍により不幸な転帰をとった.臨床経過および諸検査の結果より, 転移性多形腺腫metastatic pleomorphic adenoma症例と考えた.骨・肺に転移し, 原発巣手術より, 僅か1年4ヶ月余りで死亡した, 文献的に渉猟し得たなかでも最短の稀有な症例であった....

| AbstractList | A 25-year-old man of extremely progressive pleomorphic adenoma was reported. First, a huge tumor arising in the parapharyngeal space was surgically removed by using the cervical-parotid approach. Nine months later, a cervical vertebral tumor at C 5/6 was found. Twelve months later, this metastasized tumor was extracted. The histological examination showed benign pleomorphic adenoma, which was similar to that of the initial tumor. None of the malignant pathological findings were observed in both histological sections. Sixteen months later, he died because of the development of extensive tumors in both lungs. Based on the rapid clinical course, metastasis to bone and lungs, and histological results, we diagnosed this case as a rare, progressive metastatic pleomorphic adenoma arising in a very uncommon region.
