経十二指腸的乳頭切除術を施行した腎移植後の十二指腸乳頭部腺腫の1例
症例は54歳，男性．糖尿病を原疾患とした慢性腎不全にて45歳時より血液透析導入され，46歳時に父親をドナーとした生体腎移植を受けた．以後，当科にてフォローしていたが，今回，上部消化管内視鏡検査にて十二指腸乳頭部に腫瘍性病変を認めた．術前生検診断は腺腫であったが，腎移植後8年であり免疫抑制剤を長期内服していることから腺腫内癌の存在も否定できず，経十二指腸的乳頭切除術を施行した．術後病理診断は腺腫であった．術後経過は良好であり，移植腎の機能低下も認めなかった．十二指腸乳頭部腺腫は前癌病変と考えられ積極的な切除が必要であり，本術式は本症例のようなハイリスク症例に対しては有用な術式と考えられる．...
