B 型急性大動脈解離破裂に対する部分弓部下行大動脈置換術
症例は67歳男性．背部痛で発症し，造影CTにてB型急性大動脈解離と左血胸を認め，破裂の診断にて緊急手術を施行した．左大腿動脈から人工心肺を開始し，左第4肋間開胸でアプローチした．遠位弓部に破裂を認め，超低体温循環停止下のopen proximal法にて左鎖骨下動脈近位から下行大動脈までの部分弓部下行大動脈置換術を施行した．術後，脳梗塞を発症したがリハビリテーションにて回復した．B型急性大動脈解離破裂は死亡率の高い疾患であり，特に弓部置換術による救命例は検索した範囲で他になかった．術中の脳合併症は左開胸下の弓部大動脈手術の大きな問題点であり，緊急手術といえども脳保護のための適切な戦略が必要である...
