膵粘液癌による腹膜偽粘液腫の1例
膵粘液癌の発生頻度は膵腫瘍全体の0.6％で稀な腫瘍とされている．今回，われわれは膵粘液癌に由来する腹膜偽粘液腫の1例を経験したので文献的考察を加え報告する．症例は78歳の男性．右側腹部痛の精査目的に当院を紹介された．CTで膵体尾部に約6cm大の嚢胞性腫瘍を認め，MRIでも同様に，内部に隔壁を有する壁の厚い嚢胞性腫瘍を認めた．また，網嚢内や肝表面，右傍結腸溝に液体貯留像を認めた．膵管内乳頭粘液性腺癌の網嚢内への穿破と診断し，膵体尾部切除術を施行した．開腹所見では，腫瘍周囲の網嚢内および肝周囲，Douglus窩に粘液の貯留を認め，腹膜偽粘液腫と診断した．病理組織学的には粘液癌と診断された．術後，膵...
