気道閉塞および上大静脈症候群で発症した巨大梅毒性胸部大動脈瘤の1例

Abstract 梅毒性大動脈瘤は抗生剤による治療が確立した現在では稀な疾患であるが，巨大な場合には周囲臓器を圧排して発症することがある．今回われわれは，気道閉塞および上大静脈症候群で発症した巨大梅毒性胸部大動脈瘤を経験し，良好な結果を得たので報告する．症例は62歳，男性．呼吸困難を主訴に前医に救急搬送され，CT検査で上行から弓部にかけて最大径90 mmの嚢状大動脈瘤を認めた．圧排により両側主気管支は高度に狭窄し，上大静脈は完全閉塞していた．検査中に気道閉塞から呼吸停止となったが蘇生され，気管内挿管後に当院転院搬送となった．人工呼吸器管理とするも呼吸状態が不安定なため緊急手術を行った．大動脈瘤は胸骨裏面に達していたため，大腿動静脈バイパスで体外循環を開始して冷却し，循環停止下に開胸した．大動脈瘤と周囲臓器の癒着は強固であった．脳分離体外循環を用いて上行大動脈から腕頭動脈遠位まで1分枝再建を伴う上行部分弓部大動脈置換術を行った．術前の梅毒血清反応は強陽性で，病理学的所見からも梅毒性大動脈瘤と診断した．術後のCT検査で両側主気管支の狭窄は解除されていた．術後benzyl penicillinを14日間投与し，大きな合併症なく退院となった．
