中咽頭血管肉腫の1例
血管肉腫は軟部肉腫の2％を占める稀な軟部悪性腫瘍である。 頭頸部における血管肉腫の報告の多くは頭部の皮膚原発で，中咽頭の発症は極めて稀である。今回初診時より多発遠隔転移を認めた中咽頭血管肉腫の1例を経験した。 血管肉腫の予後は非常に不良である。特に遠隔転移を伴うものでは生存期間中央値も3か月程度と報告され，エビデンスを伴った明確な治療方針はない。 本症例においてはパクリタキセルの投与により中咽頭病変は消退した。遠隔病変も縮小していたが，新規遠隔病変出現，増大を認めたため，パゾパニブに変更した。パクリタキセルの無増悪生存期間は6か月，全生存期間は17か月であった。文献的考察も踏まえ症例報告する。...
