大動脈解離術後遠隔期に人工血管高度屈曲による溶血性貧血をきたした1例

大動脈解離術後遠隔期に人工血管高度屈曲による溶血性貧血をきたした1例を報告する.症例62歳,女性.10年前に急性大動脈解離を発症し,弓部大動脈人工血管置換術を施行されている.数カ月前から労作時呼吸困難が出現,2カ月前に溶血性貧血を指摘された.数回輸血を行ったが効果は一時的で,諸検査から人工血管屈曲による溶血が疑われたため再手術適応となった.中等度以上の大動脈弁閉鎖不全と左鎖骨下動脈閉塞も合併していたため,手術は屈曲部の人工血管をトリミング,再縫合とともに,大動脈弁置換術と左鎖骨下動脈再建も併施した.術後経過良好で,溶血所見も消失した....
