脾sclerosing angiomatoid nodular transformationの1例

症例は40歳，男性．検診の腹部超音波検査で脾腫瘤を指摘された．自覚症状はなく，血液検査ではCRPの上昇を認めた．腹部造影CTで脾内に8cm大の八頭状の腫瘤を認め，動脈相では腫瘤の辺縁が淡く造影された．門脈相から平衡相にかけて内部の一部が車軸様に造影されるものの，内部の多くの領域では造影効果を認めなかった．MRIで腫瘤はT1強調像，T2強調脂肪抑制像ともに低信号を示し，PET-CTでは腫瘤に一致してFDGの集積を認めた．画像検査での良悪性の鑑別は困難であり，診断と治療目的に腹腔鏡下脾臓摘出術を施行した．摘出標本では，脾臓に8cm大の境界明瞭な充実性腫瘤を認め，病理組織学的検査では，異型の乏しい血...

Abstract 症例は40歳，男性．検診の腹部超音波検査で脾腫瘤を指摘された．自覚症状はなく，血液検査ではCRPの上昇を認めた．腹部造影CTで脾内に8cm大の八頭状の腫瘤を認め，動脈相では腫瘤の辺縁が淡く造影された．門脈相から平衡相にかけて内部の一部が車軸様に造影されるものの，内部の多くの領域では造影効果を認めなかった．MRIで腫瘤はT1強調像，T2強調脂肪抑制像ともに低信号を示し，PET-CTでは腫瘤に一致してFDGの集積を認めた．画像検査での良悪性の鑑別は困難であり，診断と治療目的に腹腔鏡下脾臓摘出術を施行した．摘出標本では，脾臓に8cm大の境界明瞭な充実性腫瘤を認め，病理組織学的検査では，異型の乏しい血管腫様結節と線維成分の増生を認めた．CD31・CD34・CD8を用いた免疫組織化学検査では異なる3種の血管成分を認め，sclerosing angiomatoid nodular transformation （SANT）と診断した．これまでSANTの本邦報告例は27例であり，自験例を加えた28例の臨床的特徴を検討し報告する．
