ネコひっかき病の臨床的検討
血清学的にBartonella henselaeの感染が確認されたネコひっかき病(CSD)63 例の臨床症状,検査所見,治療経過について検討した.CSD 患者の年齢は0 歳から83 歳に分布し,平均年齢は35.0 歳であり,男女比は 1 : 1.2 で,女性に多かった.CSD の発生には季節性がみられ,夏から秋に多く,10 月がピークであった.CSD の感染源は61 例(96.8%)がネコであり,そのなかで39 例(61.9%)が1 歳未満の仔ネコであった.感染経路は約49.2%がひっかき傷であり,3.2%は咬傷,3.2%はネコ蚤刺傷であった.受傷部位の皮膚に硬結や膿胞などの皮膚病巣が観察でき...

Abstract | 血清学的にBartonella henselaeの感染が確認されたネコひっかき病(CSD)63 例の臨床症状,検査所見,治療経過について検討した.CSD 患者の年齢は0 歳から83 歳に分布し,平均年齢は35.0 歳であり,男女比は 1 : 1.2 で,女性に多かった.CSD の発生には季節性がみられ,夏から秋に多く,10 月がピークであった.CSD の感染源は61 例(96.8%)がネコであり,そのなかで39 例(61.9%)が1 歳未満の仔ネコであった.感染経路は約49.2%がひっかき傷であり,3.2%は咬傷,3.2%はネコ蚤刺傷であった.受傷部位の皮膚に硬結や膿胞などの皮膚病巣が観察できたのは27 例(42.9%)であり,受傷部位は上肢に多かった.リンパ節腫大は60 例(95.2%)に認められ,腋窩部が最も多く(51.7%),次いで鼠径部(31.7%),頸部(21.7%),肘関節(16.7%)の順に多かった.潜伏期の平均は18.9 日であり,治療開始からリンパ節腫大が消失するまでの期間の平均は44.2 日であった.白血球の平均は8,130/µL であり,CRP の平均は2.83mg/dL であった. |
