Abstract | 「緒言」顎変形症患者に対する外科的矯正治療は, 骨格的な不調和の解消, 顔貌の改善など, 矯正治療単独では達成することが難しい結果を期待できる一方, 術後の後戻りや偶発症などの問題を随伴する. 骨格性下顎後退症に対しては, 下顎骨単独の前方移動術を行うことが第一選択となるが, 骨格性上顎前突症では, 上顎骨の後方移動術を施行することが望ましいと考えられる. しかし従来のLe Fort I型骨切り術(LF)は下行口蓋動脈や翼状突起が存在するため, 骨片の除去が難しく, 予定した移動量の獲得が困難となることがある. また下顎骨単独による骨切り術の場合, 顎角下垂により術後の咬合が不安定になることがしばしば生じるため, 上下顎移動術により上顎骨後方部を挙上することで顎角下垂の回避, もしくは下顎骨前方移動量の減少を図る方が術後安定性が良いという報告もある. 顎変形症の術式に関しては形態だけでなく, 機能も考慮しなければならない. |
