甲状腺分化癌の取扱い

分化癌の治療方針を外来アブレーションの普及を踏まえ，我が国のガイドラインの内容を参照しつつ概説した。乳頭癌ではTNM分類によってリスクを評価し，甲状腺切除範囲を決定する。T1N0M0の明らかな低リスク群には葉切除を実施し，高リスク群に対してはアブレーションを前提とした全摘術を行う。どちらにも該当しない症例はグレーゾーンとして，合併症と再発・生命予後とのバランスをもとに術式を決定する。予防的リンパ節郭清は甲状腺切除と同一創で実施できる中央区域に留めることが多い。外側区域の潜在的リンパ節転移に対する再手術率をどこまで許容するかが問題となる。濾胞癌の術前診断は困難であり，その治療は遠隔転移を念頭に置...
