非手術積極的経過観察の適応と注意点

最近の30年間に世界的に甲状腺癌が急増している。増加したのは小さい乳頭癌のみであり，甲状腺癌による死亡は増加していない。このことから，小さい乳頭癌の過剰診断・過剰治療を警告する報告が多い。剖検でのラテント癌の頻度や甲状腺癌検診での癌の発見率と臨床的甲状腺癌罹患率の著しい乖離からわれわれは成人の微小乳頭癌の大部分はほとんど進行しない無害な癌であるとの仮説を立て，1993年から成人の低リスク微小乳頭癌に対して積極的経過観察を行ってきた。本論文では甲状腺癌検診のあり方への提言，積極的経過観察の適応と注意点および積極的経過観察の結果を手術群と対比して報告し，さらに積極的経過観察中の微小乳頭癌の生涯腫瘍...
