| Abstract | ［目的］近年の医療費の高騰、財政難を背景に、限られた医療資源の有効活用が求められている。特に費やされる医療費中で大きな割合を占める高血圧性疾患について、費用対効果を考慮した治療の効率化は重要な課題である。家庭における自己測定血圧（家庭血庄：HBP）は医療環境下で測定される随時外来血圧（CBP）に比べ、予後予測に優れているとされており、HBPの導入により高血圧診療の適正化が図られ、医療費の削減につながることが期待される。本研究では、CBPに基づいた高血圧診断・治療がHBPに基づいた診断・治療に移行した場合の主治医の診療行動および患者の受診行動の変化を推定し、それに伴う医療費の変化を推計することを目的とした。［方法］HBPを導入した高血圧・循環器疾患に関するコホー卜研究である大迫研究のデータおよび厚生労働省発表の統計資料等を移行確立の根拠として用いたディシジョンツリーを作成し、高血圧診断へのHBP導入による医療機関における主治医の診療行動および患者の受診の行動変化が生む医療経済的効果を試算した。［成績］HBP導入に伴う診療行動・受診行動の変化が与える高血圧性疾患に関する医療費への影響を推計すると、年間1兆0136億円の費用削減が推定された。その大部分は、降圧治療を受けておらずCBP高血圧かつHBP正常血圧である者が、HBPの導入により新規受診が不必要であると判断されることで、本来費やされるはずであった医療費が回避されることに起因するものであった。また、HBP導入による的確な血圧コントロールはその後の合併症の発症にも影響を及ぼすことが推測される。HBP導入により新規治療開始または治療増強される患者の50%において、収縮期血圧が10mmHg降圧したと仮定すると、合併症予防効果に伴い年間30億円の医療費が削減できると推計された。さらに、的確な血圧コントロールによる脳卒中の予防は合併症の医療費だけでなく介護費の削減にもつながることが推察され、HBP導入により新規治療開始または治療増強される患者の50%において収縮期血圧が10mmHg降圧したと仮定すると、合併症予防効果に伴い年間42億円の介護費が削減できると推計された。これら高血圧関連医療費、合併症関連医療費および介護費の削減額を合計し、高血圧診断へのHBP導入により年間1兆0209億円の費用が削減されると推定された。［結論］高血圧診断・治療へのHBP導入は非常に高い医療費削減効果があることが示唆され、今後HBPの更なる普及が望まれる。 |
