孤立性腹部内臓動脈解離症例の検討～本邦165既報告例を含めて

Abstract 要　　旨：孤立性腹部内臓動脈解離は比較的稀な疾患であり，その原因や自然歴，性差，予後などは今もって不明である．自験例7例を含む本邦からの孤立性腹部内臓動脈解離の報告172例について，その特徴，解離部位や解離長，症状，診断方法，治療法，そして予後について集計し検討した．連続自験例7例を含む本邦172例の平均年齢は55.0歳(31～89歳)であった．そのうち157例(91.3%)が男性で，女性は15例(8.7%)のみであった．急性発症の場合は何らかの腹痛を訴えた症例が128例(94.8%)に及んだ．慢性期診断例の68.8%は無症状であった．上腸間膜動脈解離と腹腔動脈解離も含めた解離の起点は，大動脈からの分岐部から平均1.8 cmで，3 cm以内で解離が起始している症例が94.6%を占めた．解離長の平均は5.3 cmであった．治療法は74.4%の患者が保存的治療を施行され，侵襲的治療を施行されたのは25.6%に過ぎなかった．死亡報告は3例あった．抗血栓薬を投与された群と非投与群とでは，偽腔の画像所見的な結果に有意な差を認めなかった．腹腔内臓動脈解離を連続7例経験し，1例を除き保存的治療で良好な経過を得た．孤立性腹腔内臓動脈解離は概して予後良好であったが，腸管虚血の所見を見逃さないことが重要である．抗血栓療法は，偽腔の血栓化に有意な影響を及ぼさないようである．
