硬膜動静脈瘻の病態と治療
DAVFに対して適切な治療を行うには, 病態と血管構築を理解することが重要である. DAVFの発症形式は, 部位やvenous drainage patternによって多様であり, 自然歴を考慮した治療計画が求められる. 画像診断法, カテーテル, 塞栓物質などの進歩に併って血管内治療を中心としたDAVFの治療成績は向上した. 2018年にはOnyxが承認され治療の幅が広がったが, DAVFに対する治療方針は一律ではなく, 症例ごとに血管解剖と血行動態を十分に理解して治療ストラテジーを検討することが重要である....
