Abstract ノーウォーク様ウィルス (Norwalk-like viruses, 以下NLVs) は, カリシウィルス科に属するRNAウィルスで, 冬季に生カキに起因する小規模な非細菌性食中毒をおこす病原体として知られている.しかしNLVsは非常に強い感染力を有し, 近年, 高齢者療養施設や学童における集団発生の原因として注目されている.東京女子医科大学付属病院において, 1999年11月から2000年4月の問に, 病院外から持ちこまれたNLVsが原因と考えられた急性胃腸炎の院内集団発生を4病棟で経験した.NLVsによる急性胃腸炎を発症した計56名 (患者32名, ナース19名, 医師5名) の患者・スタッフのうち, 46名 (患者27名, ナース15名, 医師4名) からの便60検体及び吐物 1検体について, RT-PCR法による検査を施行した.集団発生のコントロールにはスタンダード・プレコーションの徹底及び便・吐物に対する感染防御を呼びかけた. 結果: 61検体のうち, 28名40検体よりNLVsが検出された. attackrateは患者が0.19, 看護婦が0.15, 医師が0.07であった. 考察: ヒトーヒト感染が主な感染経路と考えられた. 看護婦は患者同様, 感染の危険が高かった. コントロールは難渋を極め, 感受性を有する患者の多くが罹患したところで流行が沈静化した病棟もあり, さらに厳格な防御策の適用が必要と考えられた.
