超音波ドプラ血流計による頸部内頸動脈の狭窄性病変の診断

Abstract | Bモード法と超音波パルスドプラ血流計の複合装置により頸部内頸動脈の狭窄性病変の診断を試みた.対象は陳旧性の脳梗塞または一過性脳虚血患者32名35血管である.狭窄性病変の診断は, 内頸動脈の最高流速 (VmaxICA) および総頸動脈の最高流速 (VmaxCCA) を測定して, VmaxICAおよびflow velocity ratio (VmaxICA/VmaxCCA) を使用して診断を行った.VmaxICAの2,500Hzを基準にすると, sensitivityが不十分であり, VmaxICAの2,000Hzを基準にするとspeci負cityが不十分であった.Flow velocity ratioの1.10を基準によるとsensitivity 100%, specificity 86%であった.Flow velocity ratioを使用する本方法は頸部内頸動脈の狭窄性病変の診断に有用と考えられる. |
