原発性胆汁性肝硬変におけるヒト胆汁蛋白抗原に対する細胞性免疫応答

Abstract ヒト胆汁より原発性胆汁性肝硬変(PBC)における標的自己抗原を,SDS-PAGEおよび正常ヒト胆管上皮細胞と反応する家兎抗血清を用いたWestern blotting (WB)法にて精製し,精製抗原に対する末梢単核球の幼若化反応を3H-thymidineの取り込みにて検討した.家兎抗血清の肝組織内対応抗原は胆管上皮細胞絨毛上にあり,毛細胆管には存在しない.WB法で検出される胆汁抗原成分は,2ME非還元下では分子量200kD以上の2個の蛋白バンド(B1,B2)として存在した.このうち脂肪染色陰性,糖蛋白染色陽性のB1に対する単核球幼若化反応がPBCにおいて疾患特異的に検出された(p<0.01).その幼若化細胞はT細胞分画に認められた.WB法ではB1は抗MHC class I, II抗体,M2抗体陽性PBC患者血清,抗分泌型IgA抗体,抗ラミニン抗体,抗type IVコラーゲン抗体とは反応を認めなかった.PBCの病因には,胆管上皮と抗原交差性を有する胆汁抗原B1に対する細胞性自己免疫が関与すると考えられた.
