Abstract 心タンポナーデで発症し,ステロイド・パルス療法が著効を示したSLEの1症例を報告する. 1ヵ月前より発熱,前胸部痛,多関節炎が出現,近医で上気道炎として抗生物質投与されるも無効,プレドニゾロン40mg 5日間併用にもかかわらず症状増悪するため当院入院となった.心エコーで大量心嚢液貯留が証明され,心タンポナーデを呈した活動期SLEと診断した.プレドニゾロン40mgが無効であったことからステロイド・パルス療法を施行したところ,諸症状は, 1週間で改善,免疫血清学的所見,心嚢液貯留および左心機能もこれと平行して改善した. 心タンポナーデで発症したSLEはきわめてまれであり,しかもステロイド・パルス療法を施行した例は,これまで報告がない.本例では急性期に心嚢穿刺をすることなくステロイド・パルス療法により著明な改善が認められた.したがって, SLEの診断が確定した心タンポナーデの治療にステロイド・パルス療法は,きわめて有効な手段となる可能性が示唆された.
