REM睡眠行動障害(rapid eye movement (REM) sleep behavior disorder; RBD)は,夢内容の行動化を呈する睡眠時随伴症である.長期的な経過観察で,αシヌクレイノパチーの発症が高率に認められる.将来の発症リスクの告知は,患者に動揺をもたらすものの,患者には知る権利があることから,適切な時期や方法を症例ごとに検討した上で告知し,告知後はRBDに対する生活指導と治療を行い,精神的にも支援する必要がある.またRBDは,他の検査との組み合わせによりαシヌクレイノパチーの早期診断を実現する可能性があり,病態抑止療法への応用が期待される....
