症例 治療に難渋したcarotidsinus syndromeの1例
腫瘤が確認できない早期より各種治療に抵抗性の失神発作を呈したが,化学療法にて寛解治癒した悪性リンパ腫によるcarotid sinus syndrome(CSS)の1例を経験した.症例は65歳男性.近医にて失神発作時の高度徐脈を認め,sicksinussyndromeの診断でペースメーカー(VVI)の植え込み術を施行.しかし,その後も発作が継続し当科紹介となった.頭部CT,脳波など神経学的に著変は見られなかった.発作時の血行動態モニターにて高度徐脈とともに急激な血圧低下を確認し,頸動脈洞機能検査では機能充進が疑われた.発作時の低血圧は,ペーシング(VVI)にもかかわらず持続した.混合型のCSSを...

| Abstract | 腫瘤が確認できない早期より各種治療に抵抗性の失神発作を呈したが,化学療法にて寛解治癒した悪性リンパ腫によるcarotid sinus syndrome(CSS)の1例を経験した.症例は65歳男性.近医にて失神発作時の高度徐脈を認め,sicksinussyndromeの診断でペースメーカー(VVI)の植え込み術を施行.しかし,その後も発作が継続し当科紹介となった.頭部CT,脳波など神経学的に著変は見られなかった.発作時の血行動態モニターにて高度徐脈とともに急激な血圧低下を確認し,頸動脈洞機能検査では機能充進が疑われた.発作時の低血圧は,ペーシング(VVI)にもかかわらず持続した.混合型のCSSを疑い,頸動脈造影,頸部CT,耳鼻科的検索を行ったが著変はなく,本態性のCSSと思われた.硫酸アトロピンの経口投与で一時期発作の消失を見たが約1カ月後に再発し,エフェドリンなど各種薬剤も無効であった.心房心室順次ペーシング(DDD)への植換え術も施行したが,発作の抑制はできなかった.その後,咽頭部の異和感を訴えるようになり,頸部CT再検で右上咽頭の腫瘤性病変を認めた.発作との関連を想定し,腫瘤の可及的i摘出術を施行した.肉眼的には神経原性の良性腫瘍を思わせたが,手術後も発作が継続しGaシンチにて同部位への集積像を認めた.組織診にて悪性リンパ腫の診断を得,化学療法にて残存腫瘤の消失とともに発作の寛解を見た.これは腫瘍の神経組織への間接的影響の他,直接的浸潤による可能性が推測された.悪性リンパ腫によるCSSは極めてまれであり,さらに化学療法にて寛解治癒した報告は他に見られず,興味ある1例と考えられた. |
