感染性心内膜炎術後の人工弁位に発生した Libman-Sacks 心内膜炎の 1 例

| Abstract | Libman-Sacks 心内膜炎は全身性エリテマトーデス（Systemic Lupus Erythematosus、SLE），抗リン脂質抗体症候群に合併する非感染性疣贅性心内膜炎であり，感染性心内膜炎との鑑別が重要である。今回，人工弁位に発生した感染性心内膜炎を疑い緊急手術を施行したが，最終的に Libman-Sacks 心内膜炎と診断された 1 例を経験した。62 歳女性，SLE，シェーグレン症候群に対して内服治療中であり，5 年前に黄色ブドウ球菌による感染性心内膜炎に対して大動脈弁置換術（機械弁）を施行された。今回，急性心不全で緊急入院となり，心エコーにて大動脈人工弁位に付着した異常構造物と重症大動脈弁狭窄，重症大動脈弁逆流，重症僧帽弁逆流を認めた。感染性心内膜炎の再発を疑い，大動脈弁再置換術，僧帽弁輪形成術，左室流出路形成術を施行したが，左室右室短絡と完全房室ブロックを合併した。大動脈人工弁の弁葉は異常組織が侵入し固定されており，摘出組織の病理診断で硝子化，ヘモジデリン沈着を伴う非感染性線維組織の増殖を認め，Libman-Sacks 心内膜炎と診断した。術後は早期に抗菌薬を中止し，抗凝固療法，ステロイド，免疫抑制剤を継続した。 |
