Abstract 高齢者の栄養状態からの予後予測を叙述的に総説解析した。【方法】PubMedにより2013年6月までに、{“elderly” and “older”} and “nutrition” and “outcome” をMeSHキーワードとしてサーチした。方法1-臨床研究アウトカム論文の予後期間別検討、方法2-臨床研究アウトカム論文の予後予測指標と論文発表年度の検討、方法3-アウトカム論文の系統的総説の検討。【結果】方法1-アウトカム指標と予後期間は生存 (死亡) 率、バイオマーカーが1~3年、身体機能、身体計測値が3~6ケ月を予後期間として扱った臨床研究が多かった。またこれらの指標は栄養アセスメント、栄養介入、病態別の3つの異なるカテゴリーに分類できた。方法2-方法1で得られた3つのカテゴリー別に、発表論文の時期を検討した結果、栄養アセスメント研究が2000年前後の5年間、栄養介入研究が2008年以降に集中していた。一方、病態別では脳卒中、褥瘡、Community-acquired pneumonia (CAP)、認知症などの臨床研究が見られたが論文の発表年度に一定の傾向は認めなかった。方法3-系統的総説は3本であった。これらの論文から栄養アセスメントによる栄養状態の悪化は身体計測値が身体機能の低下よりも先に現れ、栄養介入の効果も同様に身体計測値の効果が先に現れる傾向が認められた。
