Abstract 痴呆性高齢者における転倒・骨折と認知機能障害, 行動障害との関係を検討する目的で, 老人保健施設に入所した独歩可能な Alzheimer 型 (DAT) および混合型痴呆 (MIX) 患者154名 (平均年齢83.3歳) の転倒および骨折の発生状況を調査した. 認知機能は Mini-Mental State (MMS) で, 行動障害は Baumgarten らの Dementia Behavior Disturbance Scale (DBD) で評価した. さらに, DBD項目中歩行に関する問題行動を問う3項目の合計をDBD-Wとして検討に用いた. 転倒の観察期間は3カ月とし, 滑りなどによる単純なものは除いた. 観察期間中61名が転倒し, この内15名が骨折を併発した. 転倒群のMMSは非転倒群より有意に低く, 骨折を併発した群ではさらに低得点であった. 対象をMMS得点により3群に分け各群の転倒者, 骨折者の比率をみると, 得点の低い群ほど転倒率, 骨折率が高かった. この傾向は疾患別に分けても認められ, さらに各MMS得点群の中では, 常にMIX群の方がDAT群よりも転倒率が高かった. DBD, DBD-W得点は, 転倒群の方がと非転倒群より高いものの有意な差ではなく, DBD-W得点により対象を3群に分けた場合, 得点の高さは転倒率, 骨折率とも関係しなかった. 以上より, 施設入所中の痴呆性高齢者では, 認知機能障害の進行に伴い転倒の頻度が増し, 骨折の危険性も高まることが示唆された. また, 認知機能障害の程度が同等の場合, MIX患者の方がDAT患者よりも転倒し易いことが示唆された. 行動障害については必ずしも転倒, 骨折に関係しないと思われた.
