一絨毛膜二羊膜双胎の隔膜自然穿破にselective intrauterine growth restriction（selective IUGR）type IIIを合併した一例
一絨毛膜二羊膜双胎（MD双胎）の隔膜の自然穿破は非常に稀とされている．隔膜の自然穿破の際には，MD双胎に特有の合併症に加え，臍帯相互巻絡にも注意する必要がある．この度MD双胎の隔膜の自然穿破の症例を経験した．症例は27歳，G1P0，自然妊娠，MD双胎のため妊娠9週で当院を紹介受診し，超音波検査でMD双胎であることを確認した．妊娠19週の超音波検査で臍帯相互巻絡を指摘され，隔膜の自然穿破と考えられた．妊娠24週にselective IUGR type 3と診断し，臍帯相互巻絡もあることから妊娠33週で選択的帝王切開術の方針とした．妊娠32週3日，胎児心拍モニタリングでFGR児に変動性一過性徐脈を...

| Abstract | 一絨毛膜二羊膜双胎（MD双胎）の隔膜の自然穿破は非常に稀とされている．隔膜の自然穿破の際には，MD双胎に特有の合併症に加え，臍帯相互巻絡にも注意する必要がある．この度MD双胎の隔膜の自然穿破の症例を経験した．症例は27歳，G1P0，自然妊娠，MD双胎のため妊娠9週で当院を紹介受診し，超音波検査でMD双胎であることを確認した．妊娠19週の超音波検査で臍帯相互巻絡を指摘され，隔膜の自然穿破と考えられた．妊娠24週にselective IUGR type 3と診断し，臍帯相互巻絡もあることから妊娠33週で選択的帝王切開術の方針とした．妊娠32週3日，胎児心拍モニタリングでFGR児に変動性一過性徐脈を頻回に認めたため，胎児機能不全に対して緊急帝王切開術を施行し，生児を得た．術中に臍帯相互巻絡を確認し，胎盤に隔膜遺残を認めた．MD双胎の隔膜自然穿破については報告が少なく，今後の知見の蓄積が期待される． |
