口腔癌の局所再発例に対する救済手術（原著）

| Abstract | 近年，口腔癌に対する一次治療はほぼ確立されてきており，治療成績も向上している。さらなる治療成績の向上を図るには再発症例に対する治療成績を向上させる必要がある。今回は，根治治療後に局所（原発巣・頸部）再発した症例に対する救済手術症例を中心に検討を行った。対象は1999年1月から2012年12月までの間に，当科にて根治手術を行った症例および根治療法として化学放射線療法または放射線療法を選択しCRが得られた症例381例である。全対象症例381例中87例に局所再発を認め，再発率は22.8％であった。再発の状態としては，原発巣のみに再発を認めた症例が57例，頸部再発のみを認めた症例が15例，原発巣および頸部に再発を認めた症例が8例，原発巣再発に頸部転移を伴った症例が7例であった。再発症例に対して救済手術は45例（51.7％）に施行されていた。救済手術施行症例の3年生存率は71.3％，3年再再発率は30.1％であった。また，初回治療から再発までの期間（無病期間）が12か月以上であることが良好な予後因子であった。再発時には手術施行例が比較的良好な結果が得られたことから，これらを十分に考慮した手術適応を考えるべきであると思われる。 |
