会陰部子宮内膜症 自験4例の臨床病理学的検討と文献的考察

Abstract 会陰部子宮内膜症は骨盤腔を主とした他部子宮内膜症に比し,稀有と見做されてきたが,最近報告例が増加し内外文献から110例を集計した.この中本邦例は僅か5例で,いずれもこの10年間の報告である.われわれはこの数年間に4例の該当例を見出し,本症が既報告例をかなり上まわる発生状況を示すことを推測した.この経験が本論文執筆の動機である.自験例を臨床病理学的に検索し,既報告例と対比した結果,本症を次のごとく総括しえた. 既往分娩時会陰切開術をうけ,.病巣は創痕の一端乃至隣接部皮下に単発性,波動感のある暗赤色内容を透視できる径1～2cm,嚢腫状腫瘤として発生する.月経時,疼痛を伴なう腫瘤の増大がみられ,終了後退縮軽快する. 会陰切開後1～3年に発症のピークがあり,平均年齢は本邦例34歳と,他部子宮内膜症に比し,若干若年層にずれる.腫瘤の単純局所切除術によりほぼ根治が期待できるが,15%の再発の報告がある.組織学的に内膜腺および間質,新旧出血巣を証明することによって最終診断が確定する.電顕的にも周期的に変化する内膜細胞の微細構造を確認しえた. 本症の発生機序に関して,症例の大多数は,分娩時内膜の会陰切開創面への直接移植によって説明が可能で,この立場から本症の防止に結びつく,いくつかの対応策を提示した.
