Patients who were transferred to private hospital care or facilities were excluded from data analysis due to incomplete follow-up data availability. Discharge referrals included home assessments by occupational therapists, rehabilitation (including rehabilitation in the home [RITH] or community physiotherapy), increased home services to assist with personal or domestic activities of daily living or geriatrician led day therapy clinics (memory, falls or continence outpatient clinics). The number of post-discharge patient contacts at 30, 60, and 90 days by health care services were also captured. Patient follow-up data was sourced from existing hospital software systems including Emergency Department Information System (EDIS); ¡SOFT, which provides a summary of information on admissions, discharge summaries and appointments; eReferrals, an electronic referral system; as well as the occupational therapy department referral databases.
