Argues that the ahistorical self-understanding of bioethics prevents consideration of the long history of medical ethics & past critical perspectives in research on managed care. The mechanisms for managed care were invented 2 centuries ago, & moral guidance was provided by physician-ethicists who emphasized justice considerations in the allocation of resources. It is maintained that their work could provide a critical perspective on current approaches to the ethics of managed care. Formation of the basic business tools of the managed practice of medicine as a way to address the Medicare payment system is described, along with the invention of the same tools in 18th-century British medical institutions, & the crisis of trust they helped to create. Responses of physician-ethicists to the crisis are evaluated in terms of their relevance for dealing with contemporary managed practice. A critical historical perspective on contemporary managed care is related to emphasize the key ethical themes of power, integrity, & trust. These physician-ethicists' work provides a critical perspective on current approaches to the ethics of the managed practice of medicine, approaches that put strong emphasis on considerations of justice in the allocation of resources.
