The purpose of this article is to introduce a modified arthroscopic technique using the long head of the biceps tendon as augmentation for superior capsule reconstruction with fascia lata autograft.

A 30° arthroscope is used through a standard posterior portal in the right shoulder of a 67-year-old female patient in the beach-chair position. An irreparable rotator cuff tear is identified during arthroscopic examination. The fascia graft is securely fixed to the supraglenoid fossa with anchor suture and to the greater tuberosity with a suture-bridge technique. Full coverage of the rotator cuff defect by the fascia graft is confirmed posteriorly by close approximation between the posterior margin of the fascia graft and residual rotator cuff tissue. Being identified by probing, the proximal part of the long head of the biceps tendon (LHBT) is sutured to the anterosuperior part of the fascia graft.
