Abstract | To investigate the relations of circulating adhesion molecule vascular cell adhesion molecule-1 (VCAM-1) and intercellular adhesion molecule-1 (ICAM-1) with carotid artery elasticity in patients with impaired glucose regulation (IGR).
A total of 208 subjects were enrolled from January 2013 to March 2014. One hundred forty-eight were IGR patients, and 60 had normal glucose tolerance (NGT). Carotid intima-media thickness (IMT), carotid artery pressure-strain elasticity coefficient (Eρ), stiffness (β), arterial compliance (AC), and pulse wave velocity (PWVβ), as well as blood pressure, body mass index, blood glucose, blood lipids, insulin resistance index, VCAM-1, and ICAM-1 were measured and compared between IGR and NGT patients.
Eρ, β and PWVβ were significantly higher in the IGR group than in the NGT group (P<0.05), but the IMT showed no significant difference (P>0.05). VCAM-1 and ICAM-1 were significantly higher in the IGR group than in the NGT group (P<0.05). VCAM-1 and ICAM-1 were positively correlated with Eρ, β, and PWVβ and negatively correlated with AC in IGR patients.
Changes in carotid artery elasticity and endothelial dysfunction are found in patients with IGR. Early comprehensive intervention should be performed in such IGR populations.
Rechercher une association entre les taux circulant de VCAM1 (Vascular Cell Adhesion Molecule-1) et d’ICAM-1 (Intracellular Adhesion Molecule-1) avec l’élasticité carotidienne chez les patients intolérants au glucose.
Un total de 208 sujets ont été recrutés entre janvier 2013 et mars 2014, dont 148 patients intolérants au glucose et 60 témoins sans intolérance au glucose. L’épaisseur intima-média (IMT), le coefficient d’élasticité pression-déformation de l’artère carotide (Eρ), la raideur (β), la conformité artérielle (CA) et la vitesse d’onde du pouls (PWVβ) ainsi que la pression artérielle, l’indice de masse corporelle, la glycémie et le bilan lipidique à jeun, l’index de résistance à l’insuline et les concentrations plasmatiques de VCAM-1 et d’ICAM-1 ont été mesurés et les résultats comparés entre les patients avec intolérance au glucose et les sujets témoins.
Les valeurs de Eρ, β, et PWVβ dans le groupe intolérant au glucose étaient plus élevées que dans le groupe sans intolérance au glucose (p<0,05) mais l’IMT était similaire dans les deux groupes. Les concentrations de VCAM-1 et d’ICAM-1 étaient plus élevées chez les intolérants au glucose (p<0,05), en corrélation positive avec les paramètres Eρ, β, et PWVβ et en corrélation négative avec la conformité artérielle (CA).
L’élasticité artérielle et la fonction endothéliale carotidienne sont perturbées chez les patients intolérants au glucose. Ces anomalies précoces doivent être considérées dans la prise en charge de l’intolérance au glucose. |
