Intense phototoxic reactions to photodynamic therapy in immunosuppressed renal transplant patients

•2 cases of unusual adverse effects to MAL-PDT were described.•Both patients were immunosuppressed kidney transplant recipients.•The list of side effects included pronounced purpura and flu-like systemic symptoms.
