Abstract The peroneal tubercle (PT) is a bony landmark located at the lateral surface of the calcaneus. Stenozing tenosynovitis of peroneal tendons due to the hypertrophied peroneal tubercle is a condition seldom reported in the literature. Osteochondroma of the PT is even a more infrequent cause, and there are seven reported cases in the current literature. A limited number of reports described tenosynovitis or tear of both peroneal tendons due to enlarged peroneal tubercle, but no reports mention both tendons' tear related to the peroneal tubercle osteochondroma. Furthermore, none of the previous reports mentions calcaneal bone marrow edema with peroneal tubercle osteochondroma. This report aims to review the current literature and describe the clinical, radiological findings and the operative treatment of a stenozing peroneal tubercle osteochondroma accompanying the tear of both peroneal tendons and calcaneal bone marrow edema in a 50-year-old male patient.
