Abstract La prévention par la vaccination constitue un puissant moyen de lutte contre les maladies transmissibles meurtrières de l'enfant. Au Burkina Faso, un des problèmes majeurs rencontrés dans l'immunisation est le non-respect du calendrier vaccinal, ce qui rend invalide une partie importante des doses vaccinales administrées. Afin de déterminer les facteurs associés au non-respect du calendrier vaccinal au Burkina Faso, une étude transversale a été réalisée dans le district sanitaire de Boussé avec les objectifs suivants : 1) analyser les facteurs qualitatifs et organisationnels des services de vaccination liés au non-respect du calendrier vaccinal ; 2 ) analyser les facteurs communautaires associés au non-respect du calendrier vaccinal. La population d'étude était constituée des mères d'enfants de 0 à 11 mois ayant utilisé les services de vaccination du district sanitaire de Boussé entre le 1 er août 2003 et le 30 juin 2004, et les agents vaccinateurs. Les variables étudiées étaient les attributs sociodémographiques, l'organisation de la vaccination, les perceptions des mères sur l'organisation des services de santé, les compétences des vaccinateurs, la sécurité vaccinale, la connaissance du calendrier vaccinale. Au terme de l'analyse, la mauvaise organisation des services de vaccination, la faible compétence des vaccinateurs, le faible niveau d'instruction des mères, l'insuffisance de communication avec les mères, les manifestation adverses postimmunisation sont apparus comme étant les principaux facteurs de non-respect du calendrier vaccinal. Les auteurs recommandent la formation des prestataires, une meilleure organisation des services de vaccination, la mise en œuvre d'un plan de communication. In Burkina Faso, one of the main problems for vaccination is the non-respect of the vaccination timetable bringing about a significant reduction of validity of administered doses. The authors had for aim to assess reasons for the non-compliance to the timetable. A transversal study was carried out in the Boussé health district to: 1) analyze the qualitative and organizational factors of the vaccination services linked to this non compliance; 2) analyze community factors accounting for this situation. The study target group included mothers of children aged 0 to 11 months vaccinated by the vaccination team of the Boussé health district from August 1st, 2003 to June 30th, 2004, and the vaccination team. Variables covered in this study included sociodemographic factors, the organization of vaccination campaigns, the mothers' perception of health service organization, the vaccination team's experience, vaccinal safety, knowledge of vaccination timetable. The following were identified as key factors for the non-observance of the vaccination timetable: poor organization of the vaccination services, inadequate competence of the vaccination team, poor educational level of mothers, poor communication level with mothers, postvaccination side effects. The study recommends the following: training of vaccination providers, a better organization of vaccination services, and the implementation of a communication plan.
