Abstract | To evaluate applicability and efficacy CT virtual cystoscopy in detection of urinary bladder tumors.
During the period of 14 months, 17 patients with suspicion or present of some urinary bladder lesions has undergone CT virtual and conventional cystoscopy. After examination, all data were moved to the workstation for interactive endoluminal navigation. After that, radiologist analyzed transversal and virtual images without results of conventional cystoscopy and made conclusion.
Results were divided according to their basic clinical application. By using this method, all lesions over 5 mm in size were revealed. In the group of patients that were followed up for urinary bladder tumors, three patients with carcinomatous lesion were revealed. Two tumors of bladder vault that were missed on transversal scan were visualized by virtual cystoscopy. Useful additional information about tumor spread was given in two patient. One tumor inside the bladder diverticulum was detected, that was not seen by conventional cystoscopy. In two patients, endoluminal origin of mass that could not be confirmed by conventional radiologic methods, was determined.
CT virtual cystoscopy is useful method and technics that promise a lot, especially in following situations: a) follow up of bladder tumors; b) supplemental estimation of endoscopically hardly accessible regions; c) differential diagnosis between intravesical and exravesical lesions. Optimal estimation offers adequate bladder distension with patient positioned on the back and on the belly and interpretation as well as on transversal and virtual images. |

Cilj: Ispitati primenjivost i efikasnost CT cistoskopije u otkrivanju lezija besike. Materijal i metode: U periodu od 14 meseci sedamnaest bolesnika sa sumnjom ili izvesnim lezijama besike bili su podvrgniti CT endoskopiji i konvencionalnoj cistoskopiji. Besika je najpre rasirena sobnim vazduhom kroz Folijev kateter. Zatim je izvedena CT eksploracija besike. Podaci su nakon toga preneti na radnu stanicu, radi interaktivne intraluminalne navigacije. Potom je radiolog, bez informacija o rezultatima konvencionalne cistoskopije pregledao transverzalne i virtualne snimke i donosio zakljucak. Rezultati: Rezultati su bili podeljeni u skladu sa njihovom osnovnom klinickom primenom. Ovom metodom otkrivene su sve lezije vece od 5 mm. U grupi koja je podrazumevala pracenje tumora besike otkrivena su 3 bolesnika sa karcinomskom lezijom, vizualizovana su 2 tumora u projekciji besicnog svoda koja nisu bila prikazana na transverzalnim CT snimcima. Korisna dodatna informacija o stepenu rasirenosti tumora dobijena je kod dva bolesnika, uocen je jedan tumor unutar divertikuluma besike koji nije bio zapazen pri konvencionalnoj cistoskopiji, a kod dva bolesnika je odredjeno intraluminalno poreklo mase koje se nije moglo potvrditi primenom konvencionalnih radioloskih procedura. Zakljucak: CT virtualna cistoskopija je korisna procedura i tehnika koja obecava, narocito u sledecim slucajevima : a) pracenje tumora besike, b) komplementarna procena oblasti kojima je tesko pristupiti endoskopski; c) diferencijalna dijagnoza izmedju intravezikalnih i ekstravezikalnih lezija. Optimalna procena zahteva adekvatno sirenje besike sa bolesnikom u polozaju na ledjima i stomaku, kao i interpretaciju i transverzalnih i virtualnih snimaka. .
