Abstract Identifier les facteurs de risque associés aux folliculites fibrosantes de la nuque (FFN), chez les patients consultant en dermatologie à Lomé (Togo). Il s’agit d’une étude cas-témoin menée de janvier à décembre 2018. Les patients étaient recrutés en consultation de dermatologie et les témoins en consultation de dermatologie puis dans d’autres services hospitaliers. Nous avons inclus, après obtention de leur consentement, 101 patients présentant des FFN et 202 témoins appariés selon l’âge (±5 ans). L’âge moyen des patients et des témoins était respectivement de 34,9±10,7 et 35,6±11,2 ans. Les lésions les plus fréquemment observées étaient des papules (73/101 ; 72,2 %), des cicatrices fibreuses (42/101 ; 41,5 %) et des folliculites/pustules (41/101 ; 40,6 %). En analyse multivariée, le surpoids [odd ratio (OR) 4,8 ; intervalle de confiance (IC) 95 % 2,3–11,7], l’obésité [OR 99,2 ; IC 95 % 2,1–34,2], le fait de saigner quelquefois [OR 13,9 ; IC 95 % 5,1–40,0] ou toujours lors de la coiffure [OR 22,1 ; IC 95 % 6,2–40,7], la fréquence de coiffure inférieure à une fois par semaine [OR 10,1 ; IC 95 % 3,2–50,8] et le fait de couper les cheveux ras [OR 4,9 ; IC 95 % 2,8–15,5] étaient des facteurs significativement associés à la FFN. Le surpoids, l’obésité, le saignement lors de la coiffure des cheveux, la fréquence et le type de coiffure sont des facteurs associés significativement à la FFN de l’adulte jeune. Les résultats de cette étude incitent à limiter les traumatismes lors de la coiffure et de ne pas raser ou tondre les cheveux sans sabot. Le traitement des FFN étant très difficile, l’identification de ses facteurs de risque permet une prise en charge préventive adaptée. AKN is common in young adults. Excess weight or obesity, bleeding during hairdressing, and frequency and style of hairdressing are all associated factors. The results of this study stress the importance of limiting injury during hairdressing, and of refraining from shaving or cropping hair without a trimmer. Since management of AKN is extremely difficult, identification of associated risk factors allows suitable preventive approaches to be adopted.
