Summary:Background Gastric cancer is the second most common cause of death in the world. A few serum markers in patients with gastric cancer have been identified that are able to predict disease recurrence and prognosis. We hypothesise that the ratio of neutrophils to lymphocytes (NLR) to be a predictive factor for the prognosis of systemic inflammation that is easily measurable. We investigated the prognostic role of NLR in patients with non-metastatic gastric cancer. Methods This study was done between February 2011 and December 2013 in three oncology centres in Zanjan Valiasr Hospital, Tehran Imam Hussain hospital, and Tonekabon Imam Sajjad hospital. One hundred and sixty-four patients with non-metastatic and resectable gastric adenocarcinoma were included, divided into two groups: 67 patients who received neoadjuvant chemotherapy and 97 patients who received adjuvant chemotherapy. Blood samples were collected from all patients before treatment to determine the NLR. Patients were followed for a minimum of 6 months and maximum of 18 months. We compared disease-free survival at the end of the follow-up period based on the NLR with cut off point of 1.5. Findings There was no significant relationship between the duration of disease-free survival and NLR ( p = 0.117). Interpretation Although NLR did not predict disease-free survival, it is possible that NLR in patients with non-metastatic cancer might predict resectability of gastric cancer. Proof requires more comprehensive studies.
