Abstract L’évaluation de la fonction diastolique ventriculaire gauche par échocardiographie Doppler transthoracique repose sur une approche multiparamétrique incluant en particulier le rapport E/e’ mesuré en Doppler tissulaire spectral. Récemment a été proposé l’indice E/(e’xs’) combinant E/e’ à un paramètre de fonction systolique, s’, mesuré en Doppler tissulaire spectral à l’anneau mitral. Selon certaines études, E/(e’xs’) est bien corrélé au NT proBNP et à la pression télédiastolique ventriculaire gauche invasive utilisés comme marqueurs de la dysfonction myocardique ventriculaire gauche, indépendamment de la valeur de fraction d’éjection et des anomalies de cinétique segmentaire. Plus spécifiquement, E/(e’xs’) a une bonne précision diagnostique en cas de valeur intermédiaire de E/e’ (8 à 15). Un rapport E/(e’xs’) moyenné>1,6 prédirait une pression télédiastolique ventriculaire gauche invasive>15mmHg avec une sensibilité de 86 % et une spécificité de 85 %. La mesure de ce nouvel indice offrirait de plus une information pronostique supérieure à E/e’ chez les patients souffrant d’insuffisance cardiaque systolique ou à fraction d’éjection préservée et chez les patients porteurs d’une cardiopathie asymptomatique. E/(e’xs’) prédirait la récurrence de la fibrillation auriculaire après cardioversion et le remodelage ventriculaire gauche après infarctus du myocarde aigu. Des études complémentaires sont donc nécessaires pour apprécier la place de cet indice non invasif méconnu dans l’évaluation des maladies cardiovasculaires, en particulier dans l’insuffisance cardiaque à fraction d’éjection préservée. Further experimental and clinical investigation is critically needed to determine the role of this under-recognized tissue Doppler index in noninvasive assessment of cardiovascular diseases, in particular heart failure with normal ejection fraction.
