Abstract La consommation de cannabis est très répandue chez les sujets à ultra-haut risque (UHR) de psychose. Le lien causal ainsi que le lien temporel entre la consommation de cannabis et la survenue ultérieure de psychose chez les sujets UHR demeurent non concluants. Les données actuelles de la science étaient en faveur d’un risque augmenté de survenue ultérieure de psychose chez les sujets consommateurs de cannabis qui sont génétiquement prédisposés à la psychose. Ce risque serait encore plus important en présence d’une histoire familiale de psychose, d’une forte consommation et d’un âge jeune de début de consommation. Plusieurs modèles ont été cités pour expliquer le lien entre le cannabis et la survenue ultérieure de psychose ou d’états prépsychotiques : les modifications de certaines structures cérébrales induites par le cannabis, un dérèglement de l’axe hypothalamo-hypophysaire et une altération du développement neurologique normal via le système endocannabinoide. Le cannabis représente un facteur de risque modifiable de psychose. Les interventions actuelles visent à réduire ou à arrêter la consommation de cannabis afin de réduire le risque de transition vers la psychose.
