Abstract | Plusieurs approches expérimentales réparatrices des atteintes primaires de la myéline du système nerveux central (SNC) comme la sclérose en plaques (SEP) et certaines leucodystrophies sont explorées. De nombreuses études suggèrent qu’il est possible de réparer le SNC par greffe de cellules à potentiel myélinisant ou en stimulant la remyélinisation endogène. Le développement de modèles animaux a permis de démontrer la possibilité de réparer par transplantation, des lésions focales au plan anatomique et fonctionnel. Ces modèles sont également utilisés pour valider le rôle de molécules candidates en vue de stimuler la remyélinisation endogène. Néanmoins, il semble de plus en plus évident qu’au vu de l’hétérogénéité du contexte lésionnel de la SEP, l’approche thérapie cellulaire ne puisse contribuer à elle seule, de façon optimale à la remyélinisation du SNC. Les expériences futures devraient indiquer si les approches combinées de greffe et de manipulation des cellules ou de l’environnement lésionnel, présentent un avantage sur chacune de ces stratégies appliquées individuellement.
Several ways of promoting myelin repair in myelin disorders such as multiple sclerosis and certain types of leukodystrophies are currently being investigated. Numerous studies suggest that it is possible to repair the central nervous system (CNS) by cell transplantation or by enhancing endogenous remyelination. Investigations in animal models indicate that cell therapy results in robust anatomical and functional recovery of acute myelin lesions. These models are also used to explore and validate the role of candidate molecules to stimulate endogenous remyelination by activating the myelin competent population or providing neuroprotection. However, in view of the heterogeneity of the lesion environment in MS, it seems more likely that cell therapy alone will not be able to contribute efficiently to the repair of the lesion. Further developments should indicate whether combining multiple approaches will be more powerful to achieve global myelin repair in the CNS than applying these strategies alone. |
