Abstract L'axe hormone de croissance - somatomédines est un des systèmes anabolisants essentiels de l'organisme et s'est avéré en relation avec le niveau d' activité physique et de performance aérobie. Nous avons étudié les relations entre la somatomédine C/IGFI sérique, ses protéines porteuses IGFBPI et IGFBP3 et la force musculaire chez 37 gymnastes adolescents soumis depuis 1 à 5 ans à 18–20 heures d'entraînement intensif hebdomadaire ( 13 garçons, 24 filles, âge 10–15 ans, taille 1,55 ± 0,01 m, poids 45,1 ± 1,4 kg). Le bilan nutritionnel montre que ces gymnastes consommaient environ 2 670 ± 380 kCal journalières, se réparfissant de la sorte: 16% (+2) de protides, 39% (+3) de lipides et 45 % (+1) de glucides. Somatomédine C/IGFI et IGFBP3 restent dans l'enveloppe des valeurs normales, tandis que les valeurs d'IGFBPI semblent se retrouver au dessous de cette enveloppe. Au cours de la puberté l'IGFBPI décroÎt ( p < 0,01) parallèlement á une augmentation d'IGFI et d' IGFBP3. Sur l'ensemble de l'échantillon, la force isométrique de préhension est corrélée à l'IGFBP3 ( r= 0.489 p < 0,01), et à la somatomédine C/IGFI ( r=0,3451 p < 0,05), et l'IGFBPI est négativement corrélée à l'IGFBP3 ( r=−0,431 p < 0,01). Dans le sous-groupe des 13 garçons, la force isométrique de préhension est corrélée à l'IGFBP3 ( r = 0,698 p < 0,01), et à la somatomédine C/IGF 1 ( r = −0,595 p < 0,05). L' IGFBPI est négativement corrélée à la somatomédine C/IGFI ( r=−0,553 p < 0,05) et à l'IGFBP3 ( r= −0,765 p < 0,01). L'âge n'explique pas ces corrélations puisque l'IGFBP3 et la force isométrique de préhension sont totalement décorrélées de l'âge. Ces corrélations, compte tenu du rôle anabolisant du système hormone de croissance-somatomédines sur le muscle, suggèrent l'intérêt potentiel du dosage des somatomédines et de leurs protéines porteuses (notamment IGFBP3) en médecine du sport. Growth hormone and insulin-like growth factor 1 (IGFI) are a major anabolic system in the body. They have been reported to be correlated to fitness. More recently, insulin-like growth, factor-binding proteins (IGFBPs) have been shown to modulate the biological action of IGFI. In this study we aimed at evaluating the relationships among IGFI, IGFBPs and strength measurements in adolescent gymnasts submitted to a hard training program. In 37 gymnasts (13 boys, 24 girls) aged front 10 to 15 years undergoing 18 to 20 hours of training each week in a sports-study school section, IGFI. IGFBPI, and IGFBP3 were studied in comparison with measurements performed with home made ergometers designed for handgrip and adductor isometric strength measurements. Nutritional assessment showed that children were eating 2,670 ± 380 kcal/day, including 16% (+2) proteins, 39% (+3) lipids and 45%(+1) carbohydrates Serum IGFI values (girls: 31.02 ± 2.04 nMol/L; boys: 26.34 ± 2.27 nmol/L) were within the control range (fig 1), as were also IGFBP3 (girls: 4.39 ± 0.23 mg/L; boys: 4.68 ± 0.33 mg/L, fig 2). Although the normal range, for IGFBPI is less well defined, IGFBPI values (girls: 23.89 ± 6.28 ng/mL; boys: 65.6 ± 22.52 ng/mL) seemed to be lowered (fig 3). In the whole sample, a positive correlation was found between isometric handgrip strength and both IGFI ( r = 0.345 P < 0.05) and IGFBP3 ( r = 0.489 P < 0.01). This correlation is not explained by age, which is not a determinant of handgrip strength in this sample. IGFBPI is negatively correlated to IGFBP3 ( r = −0.431 P < 0.01). IGFBPI decreases during puberty (fig 4: P < 0.01) while IGFBP3 and IGFI slightly increase. In the 13 boys, isometric handgrip strength was correlated to IGFI ( r = 0.595 P < 0.05, fig 7) and IGFBP3 ( r = 0.698 P < 0.01.fig 8). A partial correlation analysis selects IGFBP3 as a better statistical determinant ( r = 0.468) than IGFI ( r = 0.13). IGFBPI is negatively correlated to IGFI ( r = −0.553 P < 0.05) and IGFBP3 ( r = −0.765 P < 0.01). Age and duration of previous practice of gymnastics were not correlated to these serum proteins and are not likely to explain these corrélations. In the sample of 24 girls, these corrélations are not significant. Thus, isometric strength (ie. a parameter involved in performance in gymnasts) is correlated with two markers of GH status: IGFI and IGFBP3. This could be explained by the anabolic action of the GH/IGFI axis on muscle. Low IGFBPI may point out that those gymnasts are not undernourished. The correlation between IGFBP3 and strength may suggest an interest for this marker of GH action in the assessment of fitness in sports medicine.
